Essential Functions
ASSESSMENT, TRANSITION PLANNING AND IMPLEMENTATION:
Develops an individualized plan with strategies and actions that address clinical, cultural, social, psychosocial, financial needs of the patient and family.
Performs systematic and timely screening and assessment according to department standards.
Bases decisions for working transition plan on factors identified from SW assessment and ongoing reassessment and in collaboration with the patient, family, physician, care management and interdisciplinary team.
Implements the plan in a safe and timely manner and with consideration for safety, possibility for readmission, length of stay, cost effectiveness and payer requirements.
Consults with healthcare providers, vendors and agencies and makes referrals as appropriate; provides Patient Choice as required, and information about community resources.
Utilizes appropriate community resources and systems to implement the plan.
Social Work Processes
Treats patients and families with respect and compassion and implements transition plan.
Able to address and solve problems in crisis situations utilizing appropriate communication, interpersonal, negotiation, conflict management and/or counseling skills.
Establishes rapport with patient, family, significant other or designated agents.
Adapts communication style, listens and responds to patient needs.
Recognizes age-specific responses to treatment.
Provides effective interventions.
Respects patient confidentiality and privacy.
Supports social, spiritual, and cultural needs of patients and families; provides counseling and support as appropriate or makes associated referrals to address patient needs.
Manages difficult or emotional patient situations.
Provides crisis intervention and/or counseling to patients, families and visitors as needed.
Follows all regulations and hospital/system policies for reporting of abuse and neglect; when assigned assists with hospital related adoptions; documents actions taken and submits reports as required.
Seeks Legal or Risk Management guidance as needed to assure full compliance with Patient Rights and Hospital/System policies.
When assigned, may orient and mentor MSW student interns and provides timely feedback to Team Leader SW regarding performance.
Demonstrates a high degree of professionalism in keeping with acceptable SW standards of practice.
Documentation
Documents relevant notes, updates and required information and in a timely fashion according to department and PPMH standards.
Documents a plan and includes pertinent information and changes with the identified plan.
Clearly and accurately documents patient input into the plan, patient choice decisions, actions taken in planning, community agency referrals and contacts, final discharge transitional plan.
Ensures that documentation is tailored to expected readers /users.
Uses correct terminology and conforms to required style and format (electronic and written).
May prepare case summary, education or other material as needed.
Outcomes And Evaluations Of Social Work Plan
Identifies expected outcomes that provide for safe and realistic transitional plan.
Considers patient adherence to the plan and the risk for readmission.
Involves patient/family and other disciplines to determine expected outcomes.
Modifies expected outcomes based on changes in the assessment of the patient and response to plan.
Includes the patient and others involved in the care in the evaluation process.
Evaluates the plan in relation to patient responses and expected outcomes.
Documents the results and any modification to the plan.
Additional Duties
Adheres to the hospital and departmental attendance and punctuality guidelines.
Performs all job responsibilities in alignment with the core values, mission and vision of the organization.
Performs other duties as required and completes all job functions as per departmental policies and procedures.
Maintains current knowledge in present areas of responsibility (i.e., self education, attends ongoing educational programs).
Attends staff meetings and completes mandatory in-services and requirements and competency evaluations on time.
Demonstrates competency at all levels in providing care to all patients based on age, sex, weight, and demonstrated needs.
For non-clinical areas, has attended training and demonstrates usage of age- specific customer service skills.
Wears protective clothing and equipment as appropriate.
