PATCH Community Health Worker (CHW) - Lead
Job Summary
This is a new supervisory position leading Community Health Workers (CHWs) serving as frontline public health workers who have a close understanding of the communities they serve. Their role is to facilitate access to health and social services, improve service delivery quality and cultural competence, and build individual and community capacity through education, support, and advocacy.
The Community Health Workers on the Patients Accessing Trusted Community Health Workers in the Hospital (PATCH) team will serve as a liaison between healthcare providers at PeaceHealth Southwest Medical Center and the community. This role involves promoting health and wellness through a true whole-person approach to patient care, assessing social determinants of health, assisting with appropriate healthcare access, and helping communities adopt healthier lifestyles by accessing resources and social service connections. CHWs play a key role in reducing health disparities and improving overall health outcomes for diverse populations.
PATCH Community Health Workers will work dynamically in the PeaceHealth Hub and community. This is a full-time, 40-hour work week position and requires two full 8-hour days minimum of working at the Community Health Hub. When not on-site at the hospital, CHW’s are expected to meet and support clients in the community as well as complete all necessary documentation in a timely manner. This specific position is a supervisory one leading the small PATCH Team.

Essential Functions
Administrative Support and Staff Engagement – 20%
- Provide support to the PATCH Team including task assignments, meeting facilitation, coaching, and documenting as needed or required.
- Assume the duties of the Community Wellness Senior Director in their absence (vacation, illness, etc.) until they return.
- Ensure clear, professional, and timely communication across teams, stakeholders, and clients to foster collaboration, minimize misunderstandings, and achieve project goals efficiently.
- Maintain high standards of professional conduct, adhering to ethical guidelines and boundaries.
- Receive referrals from Peace Health staff for patients and be able to respond to patients within an hour.
- Conduct outreach to build positive relationships with community partners and establish connections with resources to support the needs of clients.
- Help individuals access services and community resources (e.g., food assistance, housing support, mental health services) by providing referrals and supporting them in overcoming barriers to meeting their goals.
- Assist individuals in navigating the systems, follow up on appointments, track progress, and provide ongoing support. Provide coaching and support for client identified needs.
- Work closely with healthcare teams, social workers, government agencies, and community organizations to deliver comprehensive care and services.
- Collect and maintain accurate records of community health data, client progress, and outcomes.
- Obtain necessary consent forms and Releases of Information (ROIs.) Use the Care Coordination System to maintain accurate and timely records of all client interactions. Ensure all documentation complies with organizational standards and HIPAA regulations.
- Knowledge of housing first philosophy, harm reduction, and trauma informed care
- Knowledge of challenges faced by those living in homelessness
- Ability to be aware of and to respect cultural differences and health disparities
- Skilled in crisis intervention techniques
- Assessment of social determinants of health and the ability to develop person-centered goals
- Ability to build positive and trusting relationships with diverse populations, particularly people experiencing serious and persistent mental illness and substance use disorder
- Ability to establish and maintain professional working relationships
- Knowledge of effective time management techniques and ability to prioritize tasks.
- Ability to pay attention to details in documentation and record-keeping.
- Ability to manage multiple clients and programs simultaneously.
- Knowledge of basic computer skills, including familiarity with Microsoft Office Suite or Google Workspace.
- Knowledge of and comfort with virtual communication tools (Zoom, Teams, etc.).
- High school diploma or GED required.
- Certification as a Community Health Worker (or ability to obtain within a specified time frame).
- Previous experience in community health, outreach, or social services is preferred.
- Knowledge of public health issues, health education, and community resources.
- Strong interpersonal skills and the ability to connect with diverse populations.
- Ability to work independently and as part of a team.
- Familiarity utilizing an electronic health record (EHR) to document client interactions preferred.
- Bilingual (Spanish, etc.) is a plus, depending on the community served.
- Valid driver's license and reliable transportation (if applicable).
- Ability to go through Peace Health’s badging process which includes a background check, UA, and vaccinations required.
- Successfully completing all new employee onboarding training.
- Experience with data entry and using electronic health record (EHR) systems is preferred.
- Familiarity with HIPAA and confidentiality practices preferred.
- Associate or Bachelor’s degree in public health, social work, or a related field preferred.
