The Director of Value Based Clinical Care directs the ongoing development, implementation, and coordination of physician network quality and population health initiatives with the ultimate aim of enhancing the quality of patient care and outcomes in the physician network. This position will direct the management and continued growth of various programs and projects associated with quality improvement across the physician network inclusive of governmental regulatory programs and third party payer quality programs.

The Director of Value Based Clinical Care directs the ongoing development, implementation, and coordination of physician network quality and population health initiatives with the ultimate aim of enhancing the quality of patient care and outcomes in the physician network. This position will direct the management and continued growth of various programs and projects associated with quality improvement across the physician network inclusive of governmental regulatory programs and third party payer quality programs.
Required
- Bachelor's degree in Nursing, Healthcare, or related field.
- At least five years of increasing levels of leadership experience with a progressive healthcare organization in areas of clinical, value based care, physician quality improvement, or population health management.
- Excellent interpersonal, written, verbal, and team-building skills.
- Keen knowledge of the Affordable Care Act and evidence-based service delivery models that encompass care coordination, disease management, informatics, wellness, quality improvement and ancillary clinical services.
- Master's degree in Nursing, Healthcare, or related field.
- Certification in CMQ, CPHQ, Population Health Coordination, or other quality improvement methodology.
- Management/Outpatient (Physician Practice) Operations experience preferred.
- Recent healthcare consulting experience in areas of population health or value based care strongly preferred.
