SUMMARY OF RESPONSIBILITIES: The Resident Care Coordinator provides clinical leadership for nursing employees in the ALF setting and assures smooth operations of resident care. This individual is responsible for providing nursing care within the limits of the Missouri Nurse Practice Act to a specific group of residents. The individual in this role ensures the well-being of residents through quality care delivery consistent with philosophy of assisted living and state laws and regulations.

- Accepts and gives shift report.
- Makes rounds to evaluate status of residents and determine resource needs
- Inspects environment, identifies environmental problems, and initiates requests for correction of environmental problems.
- Plans and delegates assignments.
- Checks for appropriate amount and condition of equipment and supplies; orders supplies and equipment necessary.
- Identifies clinical and operational problems.
- Obtains assistance for emergencies and assists as necessary.
- Communicates problems and needs to Director of Resident Care or House Supervisor.
- Performs competency and probationary evaluations on CNAs/CMTs.
- Consults with Director of Nursing or House Supervisor regarding the need for disciplinary action of employees; participates in disciplinary action as necessary.
- Identifies, reports, and documents incidents and accidents; participates in investigation of incidents/accidents as necessary.
- Assures that care giving practices are consistent with regulatory requirements.
- Participates on committees as necessary.
- Completes records and administrative reports as needed.
- Participates in resident assessment and care planning activities; reviews and revises residents’ assessments and care plans as needed.
- Reviews, transcribes, and initiates physician orders.?
- Coordinates and participates in admissions, transfers, and discharges of residents.
- Supervises, and assists staff in delivering care.
- Gives direct care as needed.
- Documents and supervises others in the documentation of resident care.
- Communicates and coordinates care with residents and their families/significant others.
- Counts narcotics.
- Administers medications and treatments as required.
- Communicates with residents’ physicians.
- Makes rounds to assess residents and evaluate care.
- Implements, practices, and audits staff compliance with infection control measures.
- Prepares staff for new residents, assures rooms are prepared for residents.
- Provides instruction to staff as needed.
- Assures staff attend in-service education programs and meetings as scheduled.
- Attends continuing education programs to maintain competency and improve knowledge and skills.
- Promotes positive guest relations and a positive image of long-term care.
- Assures problems and complaints involving care giving services are managed in a timely and appropriate manner.
- Maintains positive working relationships between nursing staff and other departments.
- Assists in implementing the facility’s marketing plan.
- Supports the organization’s Mission and Standards. Participates in Quality Improvement.
- Adheres to the provisions of The Groves Ethical Standards, Guidelines/Compliance plan, and exemplifies high ethical standards in all activities and interactions.
- Complies with all provisions of the Health Insurance Portability and Accountability Act (HIPAA).
- Ensures that the Nursing Department maintains compliance with all federal and state regulatory and licensure requirements.
- Maintains regular, reliable, and consistent attendance at the worksite as directed by supervisor.
- Performs other duties as assigned.
- Must be a LPN with a current license in the state of Missouri.
- At least one year of nursing experience and continuing education or special preparation for gerontological and/or long-term care nursing preferred.
- Graduate from an accredited school of nursing awaiting State Board Examination.
- Must complete the Missouri approved ALF Assessor Training.
- Must possess a high degree of interpersonal relations skills and capability of relating to a variety of people and personalities. Considerable initiative, judgment, and leadership skills are also required.
- Must possess strong organizational and time-management skills.
- Must be able to read, write, speak, and understand the English language.
- Must possess the ability to deal tactfully with personnel, residents, and visitors.
- Ability to work harmoniously with other personnel.
- Ability to minimize waste of supplies, misuse of equipment, etc.
- Be able to follow instructions.
- Must be able to cope with the mental and emotional stress of the position. Resident Care Coordinator
- Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
- Must be in good general health and demonstrate emotional stability.
- Must be able to lift, push, pull, or move equipment, supplies, etc., in excess of fifty (50) pounds.
- Must be able to lift, carry, stand, bend, squat, crawl, reach, and kneel using good body mechanics for sustained periods of time.
- Exposed to body fluids, infections, odors, and behavior of residents.
- May be subject to hostile and emotionally distraught residents, family members, and staff.
- CATEGORY I: Occupational exposure to blood/body fluids likely. Use protective clothing as necessary.
