Summary

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

Duties

  • Duties include but limited to: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
  • Trains primary care teams on group visits as well
  • Develops and implements an individualized plan to achieve optimal population health and disease management
  • Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members
  • Leads Mailed FIT Program
  • Orders and interprets labs - tests - and diagnostic studies
  • Prescribes pharmacologic and non-pharmacologic interventions
  • Initiates referrals and consults to specialty services
  • - Provides education to patients and their families
  • Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes
