AuthoraCare Collective is currently seeking an RN Triage for our After-hours department supporting our Burlington and Greensboro service areas. This position is part-time: Every other Saturday and Sunday 7:00am - 5:30pm.
CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.
Job Summary
Triage calls received from patients/family members/facilities/providers to assess needs. Assign visits to team members as needed. Assess and identify patient/caregiver problems, establish patient/caregiver care goals, and identify and implement interventions for all agency programs. Work with providers to establish and implement plan of care for the patient. Work closely with members of the interdisciplinary team and communicate effectively both verbally and in writing with team members, patients, caregivers, physicians, pharmacists, and other health care professionals and community service organizations.

Requirements
Education and Experience:
- Must be currently licensed as an RN in North Carolina/ Graduate of an accredited School of Nursing
- One year of Nursing Experience in triage or case management
- One year of Hospice experience preferred.
- Two years of experience in a triage or case management position preferred.
- Understand and be committed to the Hospice philosophy of care.
- Excellent assessment and clinical nursing skills and effective interpersonal and communications skills necessary. Computer competency needed.
- Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
