Job Description
- Works with CSH operational leaders to optimize patient care visit templates to promote multidisciplinary care for patient population, focusing on aligning with fiscal programmatic sustainability
- Recognizes the special healthcare needs of patients and demonstrate the ability to meet those needs in clinical practice in a patient and family centered manner.
- Meets expectations of department staff position as required to meet customer expectations.
- Demonstrates understanding of diagnosis pertinent medical finding, rationale and goals of treatment, and medical interventions for patients through practice and written documentation.
- Based on assessment, initiates, directs and leads team to meet comprehensive needs, utilizing quality and cost effective methods. Assists in identifying and resolving ethical problems or dilemmas of individual.
- Recognizes the need for continuity and completeness of care, above and beyond needs. Communicates and incorporates relevant research findings into practice, education and consultation.
- Participates with other providers, in a schedule of coverage including taking evening, holiday and weekend call that will ensure uninterrupted ongoing timely patient scheduling and timely medical services for Employer.
- Motivates staff to improve their performance, continuously, leading to improved department performance. Maximizes potential of self and team by supporting professional growth and conducting training and orientation sessions.
- Maintains credentials and continuing education according to professional practice standards and relevant statues and regulations.
- Coordinates the development and implementation of policies and procedures that guide and support the provision of services to meet customer expectations.
- Develops and maintains cooperative relationships with colleagues as well as external contacts.
- Develops and implement quality improvement projects in the practice setting.
- Completes pertinent clinical documentation at departmental and organizational standards.
