Job Summary
The Infectious Disease Post-Acute Nurse Practitioner (APRN) provides specialized medical care to patients in skilled nursing facilities. Responsibilities include performing patient rounds, communicating effectively with facility staff and patients, and ensuring accurate assessment, diagnosis, treatment, consultation, and follow-up care.
Previous Infectious Disease (ID) Experience Is Required.

Job Summary
The Infectious Disease Post-Acute Nurse Practitioner (APRN) provides specialized medical care to patients in skilled nursing facilities. Responsibilities include performing patient rounds, communicating effectively with facility staff and patients, and ensuring accurate assessment, diagnosis, treatment, consultation, and follow-up care.
Previous Infectious Disease (ID) Experience Is Required.
Schedule & Facilities
- Encompass Clermont: 3 days per week (flexible hours)
 - 5–10 patients per day
 - Healthcare and Rehab of Sanford: Minimum 1 day per week
 - 5–10 patients per visit
 - Opportunity to grow census into a full-time position
 
- Perform patient examinations and record medical histories
 - Follow charting and documentation protocols
 - Collaborate with physicians and interdisciplinary teams to develop care plans
 - Order and interpret diagnostic tests or imaging as needed
 - Analyze test results, establish diagnoses, and discuss findings with patients
 - Prescribe and manage appropriate treatments and medications
 - Educate patients on health maintenance, diet, hygiene, and disease prevention
 - Administer vaccines and immunizations as required
 - Refer patients to specialists when appropriate
 - Report contagious diseases and deaths to appropriate authorities
 - Perform other duties as assigned
 
- Current, active State of Florida APRN license in good standing
 - Current National Board Certification
 - Current DEA license
 - Infectious Disease and Post-Acute Care experience required
 
- Strong written, verbal, and electronic communication skills
 - Initiative, creativity, and sound problem-solving abilities
 - Ability to collaborate with physicians, nursing staff, and facility teams
 - Experience with electronic medical records (EMR) systems
 - Bilingual (English/Spanish) preferred
 
Reports to the Supervisory Physician and Post-Acute Executive Director
