Scion Staffing has been engaged to conduct a search for an Program Manager for a mission-driven community health organization focused on improving access and outcomes for individuals with complex needs. This is an onsite position based in Santa Cruz County, California.
Position Overview
We are seeking a compassionate and strategic program leader to manage a dynamic care coordination initiative supporting individuals enrolled in Medi-Cal. The Program Manager will oversee daily program operations, support and supervise a collaborative team, and ensure quality, person-centered care. This role offers a unique opportunity to make a meaningful impact while helping to shape and grow an essential community service.

This is a full-time, exempt position offering a pay range of $40-44 per hour. A per hour bilingual/biliterate differential is available after passing a language assessment. - Guide the implementation and continuous improvement of community-based care coordination programs.
- Collaborate with internal teams and external partners to ensure effective, person-centered service delivery.
- Provide supportive supervision, training, and coordination to care team staff across various programs.
- Engage in direct care management services when needed, including outreach, intake, and care planning.
- Monitor program data, reporting, and compliance while staying informed on state-level policy updates.
- Represent the program in interdisciplinary meetings and contribute to strategic partnerships and community engagement efforts.
- Bachelor's degree in social work, psychology, or a related field
- Supervisory experience
- Understanding of trauma-informed care and principles of health equity.
- Experience supporting individuals navigating complex social, behavioral, and health-related challenges.
- Ability to lead and inspire diverse teams in a supportive and inclusive manner.
- Proficiency or willingness to learn systems for electronic care coordination, reporting, and billing.
