About Telecare Oregon ACT
Telecare’s Oregon Assertive Community Treatment (TACT) program supports up to 150 adults (ages 18–59) living with serious mental illness. Our interdisciplinary team provides wraparound, field-based care — from medication management and crisis support to housing and employment services — helping members achieve stability and community connection.

What You’ll Do
- Provide field-based mental health and case management services in homes, hospitals, and community settings
- Build strong, trusting relationships to support members’ recovery goals
- Collaborate with community partners to connect members with housing, benefits, employment, and medical care
- Conduct psychosocial assessments, service plans, and crisis interventions
- Offer cognitive-behavioral and skill-based interventions that promote stability and independence
- Participate in 24-hour on-call rotation (shared among clinical team)
- Provide transportation for members served as needed (mileage reimbursed weekly at IRS rate)

Required
- Master’s degree in psychology, social work, counseling, or related behavioral science field
- Registered as a Qualified Mental Health Professional (QMHP) in the State of Oregon
- Current CPR, CPI, and First Aid certification (within 60 days of hire)
- Valid driver’s license, auto insurance, and willingness to use personal vehicle (mileage reimbursed)
- At least 18 years old
- Bilingual (Spanish or other language)
- Experience with housing, dual diagnosis, or vocational rehabilitation
- Familiarity with recovery-oriented care and community-based behavioral health services
Expected starting range: $26.46 – $32.69/hour
