Hourly Pay Range (Minimum - Midpoint - Maximum):
$77.910 - $100.530 - $112.590
The stated pay scale reflects the range that Sharp reasonably expects to pay for this position. The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant’s years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.
What You Will Do
Assists inpatient physicians in managing the care of the Diabetes patients. Collaborates with medical team, hospitalists, and other health team members to ensure maintenance of Diabetes patient care standards and continuity of care. Performs inpatient extended role procedures as defined by Standardized Procedures to facilitate the timely and appropriate care process of the Diabetes population. Collaboratively sets parameters and guidelines for continuum of care with the multidisciplinary team. Educates patients and families regarding expected course for inpatient stay. Participates in continuing education programs, current literature reviews to maintain and upgrade skills and knowledge.

Required Qualifications
- Master's Degree in Nursing
- 2 Years experience as a Diabetes Nurse Educator.
- California Registered Nurse (RN) - CA Board of Registered Nursing
- California Nurse Practitioner (NP) Certificate - CA Board of Registered Nursing
- AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association
- Certified Diabetes Care and Education Specialist (CDCES) - Certification Board for Diabetes Care and Education -PREFERRED
- DEA Controlled Substance Registration - U.S. Department of Justice -PREFERRED
- Certified Diabetes Care and Education Specialist (CDCES) - Certification Board for Diabetes Care and Education is required within 6 months of hire. Department to track and maintain this certification.
- Consultation Demonstrates collaboration with physicians, staff, patients, families, and all other internal and external customers to ensure patient and hospital outcomes. Coordinates with staff in setting patient centered goals to meet outcomes. Assists staff with implementation of patient improvement processes. Conducts rounds to evaluate care, identify problems, and initiate actions per Standard procedures or physician orders. Communicates with all levels of personnel and patient/family to coordinate and facilitate timely patient care and discharge. Facilitates the ethical decision-making in patient care. Anticipates patient outcomes and takes appropriate action to ensure or minimize deviations in care. Serves as a consultant to individuals and groups within the profession, lay communities, and other hospitals/institutions. Provides ideas and strategies to Diabetes Services administration for improvement in quality, financial, or customer service.

- Patient Care Assessment: Performs ongoing assessment based on advanced nursing practice principles and specialty knowledge of patient population within scope of NP practice. Orders and interprets appropriate diagnostic testing. Discriminates between normal and abnormal findings and communicates using appropriate venues. Identifies problems and coordinates with physicians for resolution. Initiates referrals to appropriate resources. Conducts daily rounds and facilitates efficient movement of the patient through the healthcare system, including assistance with discharge planning. Implements measures to maintain and promote health. Implements and evaluates patient plan of care. Orders treatment modalities as guided by standardize procedures and physician consultation. Performs standardized procedures as clinically directed and indicated.
- Professional Development Recognizes self-development needs and seeks out resources or experiences that promote professional growth. Member of professional organization related to specialty. Participates in activities that promote the nursing profession, specialty area and advanced practice nursing.

- Professional Competency Certified Diabetes Care and Education Specialist (CDCES) - Certification Board for Diabetes Care and Education is required within 6 months of hire. Department to track and maintain this certification.
