About The Role
The Physical Therapy Assistant (PTA) supports the delivery of high-quality rehabilitative care to Seen Health PACE participants by carrying out treatment plans designed and supervised by the Physical Therapist. This role helps participants maintain mobility, function, and independence across clinic, center, and home-based settings. The PTA also contributes to equipment upkeep, quality improvement efforts, and day-to-day operational support of the therapy team.

About The Role
The Physical Therapy Assistant (PTA) supports the delivery of high-quality rehabilitative care to Seen Health PACE participants by carrying out treatment plans designed and supervised by the Physical Therapist. This role helps participants maintain mobility, function, and independence across clinic, center, and home-based settings. The PTA also contributes to equipment upkeep, quality improvement efforts, and day-to-day operational support of the therapy team.
Responsibilities
Clinical Support & Treatment Delivery
- Provide physical therapy treatments under the supervision and direction of the Physical Therapist (PT), ensuring all interventions follow the participant's individualized care plan.
- Implement therapeutic exercise programs, mobility training, strengthening routines, balance activities, and other interventions geared toward improving functional outcomes.
- Deliver modality-based treatments—including ultrasound, electrical stimulation (e-stim), and thermotherapy (hot/cold treatments)—in accordance with established protocols and safety standards.
- Observe and monitor participant performance during therapy sessions; document progress and promptly report any changes in functional status, health condition, or safety concerns to the supervising PT.
- Conduct scheduled home visits for treatments, mobility assessments, and check-ins, ensuring continuity of care for participants in their residence or alternative care settings.
- Provide training and education to participants and caregivers to support safe mobility, fall prevention, and adherence to therapeutic home exercise programs.
- Assist with the maintenance, cleaning, and organization of therapy equipment to ensure a safe and effective treatment environment.
- Identify equipment needs, repairs, or replacement considerations and communicate them in a timely manner.
- Participate in quality improvement (QI) initiatives aimed at enhancing care delivery, efficiency, and participant outcomes within the PACE program.
- Support administrative tasks such as documentation, scheduling coordination, tracking participant progress, and maintaining compliance with organizational and regulatory standards.
- Current Physical Therapy Assistant (PTA) certification/licensure in the state of California.
- Experience working with older adults, long-term care, or PACE populations preferred.
- Strong communication, observation, and clinical documentation skills.
- Ability to work autonomously within a team-based, interdisciplinary care environment.
Los Angeles required. Ability to commute to Alhambra required.
