Regal Care at Glen Ridge is currently seeking a Social Worker. The Social Worker will play a key role in ensuring the emotional and psychosocial well-being of residents. This position involves collaborating with residents, families, and interdisciplinary team members to address individual needs, provide counseling, coordinate discharge planning, and support residents’ overall quality of life.
Responsibilities:
Conduct psychosocial assessments for new admissions and ongoing residents.
Develop and implement individualized care plans in coordination with the interdisciplinary team.
Provide counseling and emotional support to residents and families.
Assist with discharge planning and coordination of community resources and services.
Serve as a liaison between residents, families, and facility staff.
Document all assessments, interventions, and care plans accurately and timely.
Advocate for residents’ rights and ensure compliance with state and federal regulations.
Qualifications:
Bachelor’s Degree in Social Work (BSW) required; Master’s Degree (MSW) preferred.
Current NJ Social Work license (LSW or LCSW) required.
Experience In Long-term Care Or Sub-acute Rehabilitation Setting Preferred.
Strong interpersonal, communication, and organizational skills.
