- Help the patient and family maintain their maximum level of comfort and coping
- Acts as Case Manager and assumes responsibility to coordinate patient care for assigned caseload.
- Completes an initial, comprehensive, and ongoing comprehensive assessment of the patient and family to determine needs.
- Develops an individualized care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and family in the planning process.
- Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains/receives physician’s orders as required. Communicates with community health related persons to coordinate the care plan.
- Provides health care instructions to the patient as appropriate per assessment and plan.
- Attends interdisciplinary group meetings
- Participates in on-call duties as defined by the on-call policy
- Supervises ancillary personnel and delegates responsibilities when required.
- Registered Nurse (RN) with Indiana Licensure in good standing
- 1 year of recent nursing experience, prefer med-surg, acute care settings
- Must be willing to learn, willing to be taught and comply with regulations to excel as a Case Manager coordinating patient care for assigned caseload.
- Ability to learn an electronic medical record (EMR) system to complete timely and accurate documentation
- Ability to demonstrate excellent communication skills with patients, families, physicians, colleagues and community partners
- Must be willing to drive in service area, have reliable transportation, automobile insurance, and a valid driver’s license.
- Participates in on-call duties as defined by the on-call policy.
