Starting salary range between $55,640 - $67,695 based on experience.
Assists residents and families in adjusting to physical, personal psychological and environmental difficulties which may interfere with quality of life. Provides guidance and advice to medical and administrative personnel regarding the impact of psychosocial factors on the treatment and progress of residents. Services include advising family care givers, providing patient education and counseling, and making referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to improved health.

Essential Duties
- Collaborate with other professionals to evaluate patients' medical or physical condition
- Refer patient, client, or family to community resources to assist in recovery from mental
aid, housing, job placement or education.
- Counsel residents and resident families to help them overcome dependencies, recover
- Utilize consultation data and social work experience to plan and coordinate client or

- Collaborates with residents, families and staff regarding the development of an
