Speech Language Pathologist - $5,000 BONUS at Azza HealthCare Agency

We are seeking a Speech Language Pathologist (SLP) to join our home health team, providing skilled speech therapy services in patients' homes.

• Deliver care aligned with state practice acts, patient plans, and interdisciplinary collaboration.
• Evaluate patient abilities and develop personalized treatment plans for communication and swallowing disorders.
• Document sessions, educate patients and caregivers, and adjust care as needed.
• Participate in team meetings and maintain accurate records.
• Complete evaluations and care plans per state guidelines and provide in-service training when requested.
  
  

Qualifications: Current Oregon license in Speech Language Pathology, CPR certification, valid driver’s license, proof of insurance, and reliable transportation.
