The Wound Care Nurse Practitioner (NP), supervised by the CMO, provides specialized wound care services across home, nursing home, and assisted living settings, collaborating with patients, families, and healthcare teams.

• Providing onsite assessments, developing care plans, and educating patients and caregivers for optimal wound healing.
• Supervising nurses and medical assistants, performing advanced wound procedures, and ensuring documentation compliance.
• Requires a Master’s or Doctoral degree in Nursing, active NP licensure, wound care certification within 6 months, and at least 2 years of relevant experience.
• Position demands physical effort, fieldwork travel, and adherence to safety protocols, with a valid driver’s license and reliable transportation.
