The Nursing Home Wound Care Advanced Practitioner (AP) is a skilled clinical professional, either a Nurse Practitioner or Physician Assistant with specialized wound management training, responsible for assessing, treating, and preventing wounds in long-term care residents.
They aim to reduce complications, improve care quality and outcomes, enhance the patient and family experience, decrease preventable hospitalizations, and lower costs.
Key duties include comprehensive assessments, developing individualized care plans, performing advanced wound treatments, collaborating with healthcare teams, implementing prevention strategies, educating staff, maintaining detailed documentation, participating in quality improvement activities, and ensuring regulatory compliance.
The AP serves as a clinical resource, advocates for residents, and engages families, all while demonstrating strong communication, interpersonal, leadership, and technological skills. Qualifications include a relevant degree, at least 2 years of experience (long-term care preferred), board certification, and specialty certification in wound care. The role is full-time with benefits, requiring in-person work.
