Key Responsibilities:

• Record and manage information on inquiries, admissions, and discharges.
• Coordinate and facilitate the admissions process, including completing necessary paperwork.
• Communicate admission updates to facility representatives.
• Maintain accurate monthly census records in collaboration with the Business Office.
• Provide regular reports on referral leads, admissions, and discharges.
• Work closely with Social Services, Nursing, and the Business Office on bed availability.
• Evaluate referrals for admission candidacy, including home or community agency visits as needed.
• Maintain relationships with referral sources and notify hospitals of bed availability.
  
  

Qualifications:

• High school diploma or equivalent.
• Experience in admissions, preferably in long-term care (LTC) or skilled nursing facility (SNF).
• Positive, energetic, and creative attitude.
• Ability to work independently or collaboratively.
• Proficient with computers.
