Position Summary

The Home Health Field RN Case Manager provides skilled nursing care, coordinates multidisciplinary services, and manages the overall plan of care to ensure quality outcomes and patient satisfaction.Shape

Key Responsibilities

Comprehensive Assessment: Performs initial and ongoing patient assessments to identify clinical needs, establish homebound status, and determine the appropriate plan of care.

Care Plan Development & Management: Creates, implements, and updates individualized care plans in collaboration with the physician and interdisciplinary team.

Direct Patient Care: Delivers skilled nursing services such as wound care, IV therapy, medication management, and chronic disease management.

Care Coordination: Oversees all disciplines involved in the patient’s care, ensuring services are provided in the correct sequence and frequency.

Patient & Caregiver Education: Provides teaching on disease processes, medications, treatments, and self-care strategies to promote independence and safety.

Clinical Oversight: Supervises and mentors LVNs and home health aides, ensuring adherence to care plans and clinical standards.

Communication & Collaboration: Maintains regular communication with physicians, therapists, and other care team members to ensure coordinated, patient-centered care.

Documentation & Compliance: Completes accurate, timely documentation in accordance with Medicare, Medicaid, and agency requirements.

Outcome Monitoring: Tracks patient progress toward goals, addressing changes in condition promptly to prevent hospitalizations and improve clinical outcomes. Shape
