- Completes daily reports as directed
- Transport charts for appointments to and from each respective Wellness Nursing office to the Hansen Clinic.
- As needed: Bring additional requested charts to the offices or Hansen Clinic. Responsible for transporting pharmacy medications and lab specimens
- Periodically and at the end of the clinic day, returns charts to the respective nursing offices/stations.
- Daily, file/scan all resident reports into their charts; stock charts with progress notes, physician order forms, and other forms.
- File all resident charts at the end of each day.
- Prepare new resident charts/thins existing charts.
- Notify resident of weekly appointments with written notices: delivery of notices. Assist with resident appointments as needed.
- Monthly, prepare resident annual physical information and deliver packages to their mail slots.
- Make “Blue” cards as needed for all residents.
- Use EMR to Print residents face sheets and other medical records.
- Process medical records requests and keep appropriate logs. Email updates regarding medical record requests as needed
Experience: Long-term care experience preferred but not required.
Qualifications
- Current certification as nurse's aide with the Illinois Department of Public Health preferred but not required
- Medical records certification preferred but not required
- Reading, writing and comprehension skills needed to use medical terminology on the job.
- Ability to do simple arithmetic.
- Good communication skills.
- Interest in working with older adults.
- General knowledge of the needs of geriatric patients.
- CPR certification, recommended
