Schedule: Full Time- (8am-4pm) - Monday to Friday.

Essential Duties

  • Completes daily reports as directed
  • Transport charts for appointments to and from each respective Wellness Nursing office to the Hansen Clinic.
  • As needed: Bring additional requested charts to the offices or Hansen Clinic. Responsible for transporting pharmacy medications and lab specimens
  • Periodically and at the end of the clinic day, returns charts to the respective nursing offices/stations.
  • Daily, file/scan all resident reports into their charts; stock charts with progress notes, physician order forms, and other forms.
  • File all resident charts at the end of each day.
  • Prepare new resident charts/thins existing charts.
  • Notify resident of weekly appointments with written notices: delivery of notices. Assist with resident appointments as needed.
  • Monthly, prepare resident annual physical information and deliver packages to their mail slots.
  • Make “Blue” cards as needed for all residents.
  • Use EMR to Print residents face sheets and other medical records.
  • Process medical records requests and keep appropriate logs. Email updates regarding medical record requests as needed

Education: High school diploma or equivalent (GED) required.

Experience: Long-term care experience preferred but not required.

Qualifications

  • Current certification as nurse's aide with the Illinois Department of Public Health preferred but not required
  • Medical records certification preferred but not required
  • Reading, writing and comprehension skills needed to use medical terminology on the job.
  • Ability to do simple arithmetic.
  • Good communication skills.
  • Interest in working with older adults.
  • General knowledge of the needs of geriatric patients.
  • CPR certification, recommended
