Sequoia Transitional Care, a beautiful and welcoming 99-bed skilled nursing facility, is hiring a dedicated LVN Charge Nurse for our Part-Time shift. Whether you're experienced or newly licensed, we offer a supportive environment, training, and growth opportunities to help you succeed.

Key Responsibilities

• Provide direct nursing care to residents in accordance with individualized care plans
• Serve as the Charge Nurse during NOC shift, overseeing CNAs and ensuring high-quality resident care
• Administer medications and treatments as prescribed
• Monitor residents for changes in condition and report to RN Supervisor or physician
• Document all care provided accurately and timely in EMR system
• Communicate effectively with residents, families, and interdisciplinary team
• Maintain compliance with safety, infection control, and facility policies
• Respond to emergencies and follow established protocols
• Participate in shift handoffs, training, and care planning
  
  

Qualifications:

• Current California LVN license in good standing
• CPR certification
• Experience in a SNF or post-acute setting preferred, but not required
• New graduates encouraged to apply - we will train you!
• Strong written and verbal communication skills
• Computer literate and comfortable with EMR systems
• Compassionate, dependable, and a team player
• Must be able to provide recent medical clearance to work in SNF
• Must pass background check and be eligible to work in the U.S.
