Job Description

AccordCare Home Health is a licensed home care agency seeking experienced Registered Nurses (RNs) to complete Start of Care (SOC) admission visits throughout North Delray through Vero Beach.

Job Responsibilities

• Complete Start of Care (SOC) admission visits
• Provide prompt and accurate communication regarding client medical status to the Nurse Supervisor to ensure effective case management
• Travel throughout the Treasure Coast service area as needed
• Demonstrate empathy, professionalism, and confidence when interacting with clients and families
• Maintain thorough, accurate, and timely documentation
• Anticipate potential issues and appropriately resolve or escalate concerns
• Prioritize tasks and demonstrate a sense of urgency
  
  

Required Experience & Education

• Active Registered Nurse (RN) license (Florida)
• Current Level 2 clearance in the AHCA Clearinghouse
• TB test within the last 12 months or Chest X-ray within the last 5 years
• Prior home care experience required
• Excellent communication, collaboration, and customer service skills
• Ability to use an electronic health record (EHR) for documentation and care coordination
