Seeking a PRN or Full Time Physical Therapist to join the in-house rehab team at Polaris Rehab, a 105 bed skilled nursing facility in Cheyenne, WY,! New grads welcome to apply!

PT Responsibilities:

  • Evaluate, plan and administer treatment to residents and patients with temporary or long term disabilities to relieve pain, restore or improve function, and promote healing
  • Assure all treatment is delivered in accordance with an established plan of care
  • Provide clinical support and instruct patients, families, and caregivers
  • Monitor patient response to treatment intervention
  • Complete required forms and documentation in accordance with company policy and state/federal regulations

Required License/credentials:

  • Current Physical Therapy license in Wyoming & in good standing
  • Graduate of an accredited physical therapy program
