Position: Director of Social Services

Location: Lakeland, Florida

Schedule: Full-Time | 8:00 AM - 5:00 PM

Salary: $60,000 - $70,000 annually

About Us

Charming Lakes Rehabilitation is committed to providing compassionate, high-quality care to our residents. We are seeking a dedicated Director of Social Services to lead our social services department and ensure the emotional and social well-being of our residents.

Key Responsibilities

• Plan, develop, implement, and evaluate the Facility’s social services programs.
• Direct department policies, procedures, and operations, ensuring compliance with federal and state regulations.
• Conduct resident and family interviews to assess social and emotional needs.
• Coordinate discharge planning, social care plans, and resident assessments.
• Provide consultation to staff, community agencies, and families to address resident needs.
• Maintain comprehensive records, progress notes, and reports for administration.
• Participate in facility surveys and inspections, including preparing corrective action plans when necessary.
• Foster strong relationships with medical staff, community organizations, and regulatory agencies.
• Lead and mentor the Social Services Department team, including workforce evaluation and development.
• Develop and maintain a quality assurance program for social services.
  
  
  

Required Qualifications

• Bachelor’s degree in Social Work or a related human services field (sociology, gerontology, psychology, rehabilitation counseling, etc.).
• Minimum of 2 years social work experience in a healthcare setting, working directly with individuals.
• Nursing home experience required.
  
  
  

Preferred Qualifications:

• Master of Social Work (MSW) or related field.
• 1-2 years supervisory experience.
• Familiarity with PointClickCare or similar healthcare software systems.
