Lead Case Manager at Community Partnership for Families
The Lead Case Manager position is responsible for providing direct case management services while supporting coordination, outreach, and daily operational activities at the Dorothy L. Jones Family Resource Center (DLJ FRC).
This position serves in a lead capacity to support Family Resource Center staff, case management coordination, outreach efforts, participant engagement, and integrated service delivery activities. The Lead Case Manager will assist with ensuring quality service delivery, staff collaboration, case documentation support, and consistent implementation of Family Resource Center services and programming.
Under the direction of the DLJ Family Resource Center Site Manager, this position supports staff communication, case review processes, community engagement activities, and participant support strategies. The ideal candidate will possess strong organizational skills, professionalism, leadership abilities, and a commitment to supporting children, youth, individuals, and families through a strengths-based and community-centered approach.
This role will work 40 hours per week and may require occasional evening and/or weekend work.
Essential Functions May Include, But Are Not Limited To
- Provides direct case management services including intake, assessment, goal planning, referrals, follow-up, and participant support.
- Assists participants and families with accessing community resources, supportive services, and basic needs assistance.
- Maintains participant records and completes required documentation, case notes, and data entry in a timely and accurate manner.
- Assists with monitoring case documentation and conducting case reviews to support quality assurance and program compliance.
- Provides lead support and guidance to Family Resource Center staff and case management team members.
- Assist with onboarding support, workflow coordination, staff communication, and day-to-day operational support activities.
- Coordinates outreach activities, community engagement efforts, and recruitment strategies related to Family Resource Center services and programming.
- Supports daily operations and participant engagement activities at the DLJ Family Resource Center.
- Assists with preparation of monthly, quarterly, and annual program reports and outcome tracking.
- Coordinates participant services with community partners, schools, agencies, and service providers.
- Conducts workshops, orientations, family engagement activities, and community support events as assigned.
- Supports integrated service delivery and collaboration between Family Resource Center programs, community partners, and CPF staff.
- Participates in community meetings, outreach events, trainings, and collaborative initiatives.
- Assists with maintaining a welcoming, organized, and supportive environment within the Family Resource Center.
- Provides relevant data and information to CPF administration regularly and/or as requested.
- Works collaboratively with CPF staff, community partners, and stakeholders to support participant success and service integration.
- Other duties as assigned.

- Strong knowledge of community resources and supportive service systems preferred.
- Ability to work within a team environment and maintain positive working relationships.
- Strong computer skills including Microsoft Office, databases, and electronic documentation systems.
- Ability to conduct presentations and facilitate workshops in group settings.
- Ability to maintain confidentiality and professionalism in all aspects of work.
- Must provide reliable transportation, valid driver’s license, and proof of insurance.
- Must pass background clearance.
- Bilingual English/Spanish strongly preferred.
Education / Work Experience
- Bachelor’s Degree in Social Work, Human Services, Counseling, Psychology, Vocational Rehabilitation, or related field preferred.
- Associate degree or equivalent related experience considered.
- Minimum two years of experience in case management, family support services, community-based programming, or related field preferred.
- Previous experience providing staff coordination, training support, or lead responsibilities preferred.
- Experience working in multicultural settings and with underserved populations preferred.
- Experience working within community-based or neighborhood-based service programs preferred.
Physical Requirements
Employee in this position must have the ability to:
- Sit for extended periods of time.
- Stand and walk for extended periods of time, bend over, reach overhead, grasp, push, pull and move, lift and/or carry at least 30 pounds to waist height.
- Enter data into a computer terminal, operate standard office equipment, and use a telephone.
- See and read a computer screen and printed matter with or without vision aids.
- Hear and understand speech at normal levels and on the telephone.
- Work indoors in a standard office environment as well as outdoors for outreach and community engagement activities.
- Travel between CPF sites and community locations as needed.
Compensation
CPF offers a competitive salary, retirement, leave, and medical benefits package. Salary is dependent upon qualifications and experience.
