The Memphis VA Medical Center has an opening in Geriatrics, Home-Base Primary Care (HBPC) Service. The position reports directly to the Chief of Geriatrics and is responsible for direct patient care in either the home or outpatient settings. This position also offers provision of care through the supervision of residents through academic affiliation with the University of Tennessee Health Science Center (UTHSC).
The Medical Center consists of one main campus and ten community-based outpatient clinics (CBOCs). It’s a 1a complexity tertiary care facility within the VA healthcare system and located in one of the fastest growing Veteran-populated regions in the United States. The facility provides comprehensive care to over 47,000 Veterans in a 52-county area of Western Tennessee, Northern Mississippi, Eastern Arkansas, and parts of Missouri and Kentucky. The medical center includes 236 inpatient beds and a 26-bed Spinal Cord Injury Unit. It’s a teaching hospital, providing a full range of patient care services, with state-of-the-art technology, as well as extensive education and research programs.
Duties Include But Are Not Limited To
- Provide clinical input and oversight for all patient treatment plans for HBPC.
- Identifying patients with medical problems and determining the need for admission to the hospital.
- Visiting HBPC patients at home, when appropriate.
- Provide assessments, diagnosis, treatment, and outcome measures of healthcare problems.
- Initiate and adjust therapeutic regimens, perform histories and physicals, order diagnostic and therapeutic tests/studies, initiate consultations and referrals, and prescribe mediation/supplies in accordance with VA policy, VA formulary, federal & state regulations.
- Collaborating with community home health and hospice agencies as needed for HBPC patients.
- Serve as a consultant to nursing and other HBPC medical staff to provide quality care, educate patients, and assist patients in acute and chronic rehabilitation techniques.
- Eligible to serve on the medical center, VISN, national committees, and to assist in the development, evaluation, and communication of policies and standards to all healthcare disciplines.
- Execute position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex patient care.
- Plan and direct educational and clinical experience of medical students, residents, and fellows assigned to HBPC.
- Assume leadership roles in the development and implementation of the HBPC performance improvement plan.
- Advocate for HBPC with VHA leadership and the medical community.
- Participate in the selection of HBPC team members.

Requirements
- BE/EC in Internal Medicine or Family Medicine
- Additional Training in Geriatric Medicine and/or Palliative Care
- Full and unrestricted license from ANY state
- US Citizenship
- Be proficient in the written and spoken use of the English language
- Academically and clinically able to perform a broad spectrum of patient care services at the expert level
