Who We're Looking For

Copa Health is looking for a Team Specialist to join our Assertive Community Treatment team to provide community-based behavioral health services and support to individuals with Serious Mental Illness (SMI). Working with a multidisciplinary team, the ACT Team Specialist delivers services primarily in members’ homes and community settings focusing on recovery-oriented, person-centered services designed to promote stability, independence, community integration, and achievement of individualized treatment goals. This person will provide after-hours coverage and support to all persons being served on the team.

Key Responsibilities

  • Provides community-based behavioral health services and supports to individuals with Serious Mental Illness (SMI) in homes, shelters, hospitals, treatment facilities, and other community settings.
  • Works collaboratively as a member of a multidisciplinary ACT Team to facilitate recovery, promote independence, and support achievement of individualized treatment goals.
  • Conducts ongoing assessment of member strengths, needs, preferences, and progress and actively participates in the development, implementation, and review of treatment plans and crisis plans.
  • Provides rehabilitation, skill development, supportive counseling, coaching, and recovery support services designed to improve functioning and community integration.
  • Assists members in accessing and coordinating behavioral health, medical, housing, employment, educational, financial, and other community resources and supports.
  • Provides outreach and engagement services to individuals who are difficult to engage or at risk of hospitalization, incarceration, homelessness, or disengagement from services.
  • Serves as the primary contact for assigned members and maintains regular communication with members, families, guardians, providers, and community partners.

Qualifications

  • Bachelor’s degree in Behavioral Health, Social Work, Counseling, Psychology, Human Services, or a related field; OR
  • Any combination of education and behavioral health experience totaling six (6) years, including a minimum of two (2) years of direct experience working with individuals diagnosed with Serious Mental Illness (SMI) or co-occurring disorders.
  • Experience working on an ACT Team is preferred.
  • Bilingual (English/Spanish) preferred.
  • Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire.
  • Must have reliable transportation, valid AZ Driver’s License with state minimum insurance coverage and ability to meet and maintain Copa Health’s acceptable driving requirements.
  • TB Test clearance required at the time of hire.
