Care Coordinator at Community Healthcare of Texas
Community Healthcare of Texas has provided hospice and palliative care services since 1996. Our mission is to provide compassionate care for individuals living with serious illnesses while supporting their loved ones who care for them.
Community Healthcare of Texas is currently recruiting for a Care Coordinator.
POSITION SUMMARY
Serve as the hospice expert to professionally and seamlessly coordinate services and bridge the transitions for the patient to hospice services; develop long-term relationships at each facility to reinforce CHOT/Providence Hospice as the provider of choice.
ESSENTIAL FUNCTIONS
- Respond to referral sources in a timely manner; evaluate patient desire for hospice; meet with families/patients to discuss goals and explain services of hospice.
- Complete required paperwork with patients/families; facilitate patient discharge when needed by working with facility/hospital staff, DME, medications, hospice physicians, and other facility staff.
- Educate facility/hospital staff and follow up on pending patients; be available to staff and patients for questions and to build relationships.
- Communicate and provide daily feedback to admission staff on referrals.
- Through customer service and leveraging relationships, become the “hospice of choice,” whereby increasing hospital referrals.
- Attend and participate in routine staff meetings in person or via media provided as directed.
- Participate in and/or provide in-service training, attend various facility/hospital activities and perform other duties as required or assigned.
QUALIFICATIONS
- Four-year college degree preferred.
- 1-3 years of healthcare marketing experience required
- 1 year of hospice experience required.
- Medical knowledge strongly preferred (ability to read charts, know medications, etc.)
- Required to be proficient using PC including Microsoft office.
