We are seeking

Physical Therapists to provide personalized, 1:1 rehabilitative therapy in patients' homes and communities.

Key Responsibilities

• Practice independently within an interdisciplinary care team.
• Conduct comprehensive patient assessments and develop tailored treatment plans.
• Deliver rehabilitation therapy and adjust plans based on patient progress.

Qualifications & Skills

• Minimum of one year of physical therapy experience or successful completion of probation as a Rehab Intern.
• Licensed to practice as a Physical Therapist in the state.
• Valid driver's license required outside Manhattan.
