position. Actual compensation will be based on multiple factors such as

relevant experience, education and training to determine offered rates.

possible compensation at the time of posting. Position Summary The

Triage Registered Nurse (RN) plays a critical role in delivering safe,

timely, and patient centered care within a team-based community health

setting. This role manages telephone and in person triage, assesses

patient needs, provides evidence-based nursing guidance, coordinates

follow-up care, and supports providers to ensure continuity and quality

of care. The RN must effectively multitask, exercise strong clinical

judgment, and document all interactions accurately in the electronic

health record. Key Responsibilities Perform telephone, walk-in, and face

to face patient triage and assessments Provide nursing advice, determine

urgency of care, and refer patients to appropriate services, including

Emergency Department when needed Coordinate hospital discharge follows

ups and overnight triage follow ups Process medication refills and prior

authorizations Administer medications, injections, IV therapy,

treatments, and perform nursing procedures Respond to codes and provide

clinical coverage as needed Support providers during exams and

procedures Participate in Patient Centered Medical Home (PCMH)

activities, daily huddles, and care coordination Accurately document all

care and communications in the EHR May serve as Charge Nurse in the

absence of the designated Charge Nurse Qualifications Graduate of an

accredited school of nursing Current Registered Nurse (RN) licensure in

the Commonwealth of Massachusetts Current CPR certification Ability to

Maintain All Required Licensure And Certifications Bilingual Skills

preferred Required Knowledge & Skills Strong knowledge of nursing

principles, disease processes, and clinical assessments Knowledge of the

Nurse Practice Act and Code of Ethics Understanding of the nursing

process, managed care, and emergency procedures Familiarity with OSHA

and infection control guidelines Moderate computer skills, including

electronic documentation Reporting Relationship Reports to the Charge

Nurse, Clinical Supervisor, and Director of Nursing. Work Environment &

Physical Requirements Busy community health center in a downtown setting

Long hours may occasionally be required Prolonged standing and walking

required Ability to lift up to 40 pounds Visual and hearing acuity

sufficient for patient care and communication
