Certified Nursing Assistant (Home Care) | Milwaukee & Waukesha County, WI at CrewBloom

We are hiring Certified Nursing Assistants to deliver in home personal care to elderly clients according to an established Plan of Care. You will support each client's comfort, safety, and daily needs, and keep accurate documentation for the care team. This is an in person role covering the Milwaukee and Waukesha County area, Wisconsin.

Key Responsibilities

• Provide personal care tasks and related paraprofessional services in line with each client's Plan of Care
• Help with hygiene, mobility, transfers, and other daily living activities
• Support clients living with dementia, Parkinson's, or receiving hospice care
• Monitor client wellbeing and report changes or concerns promptly
• Complete accurate, timely care documentation
• Communicate clearly with clients, families, and the care team
• Travel between client homes within the service area

Requirements

• Current CNA certification
• Previous experience working with elderly clients
• Knowledge of dementia, Parkinson's, and hospice care
• Strong attention to detail in tasks, communication, and documentation
• Reliable and dependable
• Valid driver's license with a reliable, insured vehicle
