Hospice On-Call RN at KeystoneCare

KeystoneCare is a nonprofit provider of in-home healthcare services, offering home health, hospice, palliative medicine, and private-duty care across the Greater Philadelphia region. Our work is centered on delivering compassionate, patient-focused care that supports individuals and families during some of life’s most important moments.

As a Hospice On-Call RN, you will play a vital role in ensuring patients and caregivers have responsive clinical support after regular hours. This role is focused on bringing calm, skilled nursing judgment to urgent situations and helping patients remain as comfortable and supported as possible through close coordination with the broader care team.

Responsibilities

• Provide on-call hospice support to patients and families after regular hours.
• Assess urgent symptoms and coordinate timely interventions with the interdisciplinary care team.
• Respond to patient and caregiver calls with compassionate clinical guidance.
• Document on-call assessments, interventions, and outcomes in the electronic medical record.
• Coordinate emergency medication, equipment, and service needs with community providers.

Requirements

• Nursing degree required.
• 1-2 years of hospice or palliative care nursing experience.
• Current unrestricted RN license in Pennsylvania.
• Valid driver’s license and reliable vehicle for on-call patient visits.
• Eligibility to work in the United States.
• Availability for Monday through Thursday and Friday evening on-call coverage.
• Availability for Friday evening through Monday morning on-call coverage.
• Active CPR certification is a plus.
