ChristianaCare is currently seeking a Registered Nurse for a full-time position for the Riverside Primary Care Practice. In this position, the Registered Nurse will be a critical component of the care team for the population that this practice will serve. This primary care office is in collaboration with the Center for Hope and Healing and will provide coordinated care for patients with complex comorbid medical, mental health and psychosocial needs, including hospital discharge planning, ED diversion, care management and care coordination, in-person preventive medical care and evaluation, acute visits, robust virtual care and wrap around services such as social work and connection to community resources.

  • Performs Annual Wellness Visits, Advance Care Planning and Transitional Care Management call
  • Triages walk-in patients and ensures appropriate disposition
  • Answers and respond to telephone messages under the direction of the provider
  • Assists providers in examinations or procedures
  • Functions as a liaison between patients, physicians and staff to provide accurate communication
  • Assures adequate coverage within unit
  • Support the work of LPN and/or MA
  • Assist with staff education and evaluation of clinical competencies for new and existing employees (MA & LPN)
  • Assist with orientation of new personnel to ensure that they are properly trained to support patient and provider.
  • Performs chart audits or has designee assist with chart audits

Requirements: BSN required; or commitment to obtaining within three years from date of hire. RN license required in Delaware and Pennsylvania. Two years' experience as a RN required, physician office experience preferred. Previous Case Management experience preferred.
