Prairie St. John's is seeking a dynamic and talented Discharge Planner!

This position is a full-time position. The regular hours for this position are Monday, Wednesday, Thursday 12 PM - 8:30 PM. Tuesday 12 PM to 4:30 PM. Friday 8 AM - 12 PM.

Essential Job Duties:

  • The primary responsibility of the Discharge Planner is to maintain regular contact and communication with treatment team members, family members and community providers in order to ensure effective coordination and continuity of care. This includes contacting current external providers with updates on the patient's progress and anticipated length of stay, as well as, scheduling follow-up appointments
  • Establishes relationships for the patient with new providers for follow-up care, based on recommendations of the treatment team
  • Communicates findings to physicians and team members as evidenced by chart documentation, and treatment team/rounds involvement.
  • Coordinates discharge planning through team meetings, communication with patient and family, and community providers, completes assigned portion of the patient's medical record, ROIs to primary care provider and community providers to help ensure good communication and continuity of care.
  • Consistently documents discharge planning information and progress in the patient's record.
  • Provides direct services to patients and family through psycho-educational groups, family education, and individual support and education with goal of enhancing patient and family functioning.
  • Completes Discharge plan assessments with discipline specific summary and recommendations within established timelines for assigned patients.
  • All employees of Prairie St. John's are expected to adhere to safety policies, safe work practices, and other hospital policies. Employees are expected to complete mandatory training on time and attend mandatory meetings.
  • In order to meet the essential functions of this position, employees must work as scheduled.
  • All staff at Prairie St. John's may be required to work in different areas/units and varying shifts depending on the business needs to the facility.
  • Performs other duties as assigned or as requested


Position Requirements:

Bachelor's Degree in Social Work

Must be licensed or eligible for licensure as a Social Worker in the State of North Dakota

Must be highly organized and capable of multitasking in a high-stress environment

Must be knowledgeable in how to handle difficult situations in a way that is best for the well-being of the patient

Experience using Microsoft Office Products (Word, PowerPoint, Excel and Outlook)
