Responsibilities
The Care Transition Manager is responsible to make a compelling difference in the lives of our residents through providing excellence and innovation in aging services. The Care Transition Manager drives, identifies, approves, and secures new resident admissions for the Wesley Enhanced Living (WEL) Rehabilitation & Skilled Nursing Facility (SNF) within the WEL community. Create strategies and tactics to generate appropriate referrals into the facility through external marketing to hospital discharge planners and physicians. Exceed Medicare A/Managed Care census objectives; sustain 100% occupancy at all times.
Care Transition Manager essential duties and responsibilities include, but are not limited to:
External marketing to area hospitals/physicians: 50%; in-house/community-based: 50%
- Attain and exceed budgeted Medicare A/Managed Care census goals within first 3 months of role
- Strive to attain and sustain 100% total occupancy at all times within the SNF
Create strategy and implement external marketing program with the goal of identifying and developing new referral feeder relationships with all possible referring professionals (physicians, discharge planners, social workers, case managers, nurses) at area hospitals, rehabs, hospices, home care agencies, etc.
Identify influencers within the hospital and promote the WEL SNF differentiators vis-a- vis competition
Work with WEL outreach manager to target and secure new attending physicians for the facility
Interface daily with current community physicians and hospitalists to capture new referrals real time and convert to admissions
Establish daily presence at top feeder hospitals to obtain referrals and approve/secure admissions on site; seize all opportunities to promote WEL bedside to family members/residents at hospital and convert appropriate discharges into admissions
- Personally clinically approve and convert all appropriate referrals into admissions to include rapid review and decision back to hospital per required turnaround time; work in collaboration with facility NHA, DON and RNAC to review complex cases
- Drive rapid turnaround of long term care admission application and secure internal financial approvals
- Identify and seek out both short and long term admissions that can significantly improve case mix index (CMI)
Work in collaboration with WEL marketing team to plan, develop, and implement annual events, CEU programs and luncheons for hospital discharge planners/social workers, etc.
- Work in collaboration with community support staff to assure that all required admissions contracts and supporting documents with resident/POA/Physician are obtained/signed prior to admission
- Maintain Point Click Care (WEL clinical/financial database) and daily census reports.
- Maintain facility referral log, admissions history report, and attend daily morning clinical meetings to provide updates on admissions
- In collaboration with community leadership, create a 'Wow' factor tour experience for potential residents/resident representatives; train support back up team (social service staff and residential sales manager) to conduct tours
- Possess thorough ongoing understanding of delivery of clinical services at the facility with an ability to accurately articulate care needs/services to all internal and external clinical professionals
Qualifications
Experience Requirement: 3-5 years working in a clinical admissions function in a SNF with referral decision-making responsibilities and progressive direct marketing responsibilities
Education or Degree Requirements: Minimum High School Diploma - College degree in related field preferred
Licenses, Certifications, Registrations: LPN or RN preferred
