What Will Your Job Look Like?


The Eligibility Coordinator (EC) is responsible for conducting in-person interviews and conducting standardized testing with applicants to gain information to determine eligibility for ADA paratransit service. The Eligibility Coordinator is also responsible for reviewing recertification paperwork. This position's responsibility can also include: mailings, paperwork, documentation, and final eligibility determination. The Eligibility Coordinator provides clerical and administrative support to the ADA Assessment and Travel Training programs. Duties include phone calls (both outgoing and incoming calls), general clerical tasks, data entry, process picture ID's, handle numerous other customer requests, receptionist, and administrative support.



  • Maintain a positive attitude, build rapport and communicate effectively when interacting with individuals, support staff, applicants, and caregivers
  • Conduct outbound calls and answer inbound phone calls in a polite, timely manner
  • Enter Customer contact information into appropriate software system; ensure accuracy of data entries
  • Take photos of all Customers and create identification cards
  • Maintain program spreadsheets
  • Maintain daily customer scheduling for Eligibility staff
  • Anticipate needed supplies and order replacement supplies in a timely manner
  • Provide comprehensive reception coverage while maintaining a high level of professionalism
  • Ensure all staff and visitors are greeted in a pleasant and professional manner
  • Prepare and process mail daily, both outgoing mail and delivering all incoming mail to the appropriate departments
  • Create mailings for appointment reminders and recertification letters
  • Assist with printing and mailing determination letters
  • Enter all mailed information and dates into appropriate software system
  • Attend to a variety of administrative details such as keeping informed of department activities
  • Report promptly any complaints or unusual circumstances that may arise to Management
  • Check fax machine regularly, process 30 Med Request accordingly
  • Complete all documentation associated with each applicant
  • Input all information and thoroughly document each applicant appointment into Trapeze software as appropriate
  • Stay abreast of new research within the related field correlating to ADA assessments
  • Assist with updating documents and spreadsheets
  • Must conform to MTM policies and HIPAA standards
  • Participate in staff meetings with leadership and/or Client
  • Participate in initial and on-going training as required by MTM and Client
  • Complete all necessary reports, paperwork and maintain on-going applicant tracking logs and reports for MTM and Client
  • Complete appropriate determination for each applicant while considering all information gathered including the application, interview, supplemental documentation, and/or Functional Assessment, as needed
  • Conduct functional physical and/or cognitive assessment with applicants that may include:
  • Physical Functional Assessment using an outdoor course
  • FACTS
  • MMSE
  • Tinetti Balance and Gait Assessment


  • Conduct follow-up with applicant and/or healthcare professional, as needed
  • Participate in community outreach and education as needed
  • Build rapport and communicate effectively with MTM and Client personnel to include administrators, management, support staff and applicant while providing high quality customer service
  • Participate in appeals process, upon request
  • Work with management and the Client to follow through on service issues, troubleshooting problems, concerns, complaints, and training
  • Regular attendance required for assigned shifts and as outlined by MTM's attendance policy
  • Adhere to all MTM established protocols and policies

All you need is...



  • High School Diploma or G.E.D.
  • Bachelor's degree in Social Services, Rehabilitation, Recreation Therapy, Nursing, Ancillary Health Care Professionalism, Social Work, Human Services, or related field, preferred
  • Minimum of one year of professional experience working with individuals with disabilities
  • Experience conducting information-gathering interviews or assessments
  • Familiarity with ADA regulations
  • Intermediate level of proficiency or above with the Microsoft Office Suite including Outlook, Word & Excel
  • Must be authorized to work in the United States and not require work authorization sponsorship by our company for this position now or in the future
