The LPN manages care for patients and their families through the application of the nursing process utilizing independent judgment, collaboration and communication with all team members. The LPN develops a plan of nursing care and utilizes established clinical pathways which are implement by the care delivery team with the supervision of the RN. The LPN provides and delegates patient care services based upon patient needs and team member skills/abilities. The LPN evaluates patient response to treatment and nursing interventions, and if needed, revised the plan of care.
