Part-Time Nurse Case Manager - Outpatient with $2,400 Sign-On Bonus

Due to the critical service needs of our communities, we are offering a $2,400 sign-on bonus to support our mission in providing quality and compassionate care. This is a part-time role, requiring 24 hours of work each week, with flexible days across our Jefferson City, Columbia, and Eldon Offices, currently needing coverage on Monday, Wednesday, and Friday.

As a member of the exceptional Compass Health Nursing team, you'll have the opportunity to run a Medication Clinic while also acting as a case manager. The Compass Health Network Nursing Staff utilizes the “Champion” behavior to deliver outstanding service to our clientele. You will actively communicate with Clinical Doctors and Customer Relations Specialists to ensure all client needs are met.

Responsibilities

• Plan, organize, and coordinate the medication clinic.
• Be cross-trained to assist in other clinics and regions with time travel coordinated by the Supervisor.
• May receive Admin Time off, if coverage and CCBHO requirements are satisfied.
• Adapt to minimal to frequent schedule changes.
• Review medication education forms with clients.
• Assess client needs for medication refills.
• Complete all necessary documents regarding the medication clinic and psychiatric consultations for EHR.
• Administer IM medications, dispense samples, and maintain accurate logs on all medications received and dispensed.
• Assist with Prior Authorizations.
• Maintain and verify the injection list, discuss any side effects, and ensure orders are current.
• Complete vital signs for every client and coordinate pre-diabetes screening when necessary.
• Possess broad knowledge and experience with computers.

Qualifications

• High School/GED degree is required; Associate/Bachelor’s degree in nursing is preferred.
• Licensure/Certification: GPN, LPN, GN or RN that must pass the MO NCLEX exam within 90 days of graduation.
