Description

The Community Health Worker-Clinical serves as a clinical liaison between health and social service sectors to enhance service accessibility and improve the quality and cultural relevance of service delivery. This role facilitates individual and community capacity by enhancing health knowledge and independence through clinical evaluations, outreach, community education, informal counseling, social support, and advocacy efforts. You will connect health systems with community resources and provide clinical education aimed at reducing behavioral health risk factors.

This is a full-time, benefit-eligible position, working Monday to Friday, from 8:00 am to 5:00 pm. This is a Hybrid Position.

Essential Functions

  • Facilitate patient appointments by coordinating transportation resources.
  • Communicate with the Primary Care team regarding self-management barriers observed during visits.
  • Promote wellness by delivering culturally appropriate clinical health information and education to patients.
  • Encourage patients to take steps toward enhancing their health and managing their conditions.
  • Improve patient health literacy pertaining to their conditions.
  • Identify and help obtain clinical, social, or community services for patients.
  • Connect with patients post-hospitalization to follow up on care and provide necessary support.
  • Assist patients, caregivers, and families in achieving patient goals while honoring cultural practices.
  • Maintain scheduled communication with patients, both in-person and via phone.
  • Follow up on referrals sent to other providers and social services, checking patient outcomes and next steps.
  • Aid patients in navigating the MO HealthNet, Social Services, and community partnerships.
  • Build relationships with community partnerships, health, and social service providers.
  • Bridge gaps between Primary Care and FACT Community Partnership, addressing social determinants of health impacting patient well-being.
  • Attend Primary Care team meetings regularly.
  • Document in the electronic health record in a timely manner.
  • Ensure care coordination aligns with the MO Department of Social Services and MO HealthNet Division Primary Care Health Home initiative.
  • Pursue appropriate professional development opportunities.
  • Achieve designated goals and objectives within the set timeframes.
  • Follow the Clinic Exposure Control Plans concerning health risks.
  • Demonstrate respect and regard for all patients, families, visitors, and staff to ensure a professional environment.
  • Collaborate effectively within the team to help achieve the Clinic's goals.
  • Train and implement the PRAPARE assessment.
  • Collaborate with medical providers regarding significant health information.
  • Provide patient education related to medication adherence.
  • Work with clinical staff to complete condition assessments as required.
  • Participate in payer meetings as necessary.
  • Educate primary care providers on prescription benefits to maximize member advantages and adherence.
  • Collaborate with pharmacies to enhance medication adherence among the primary care population.
  • Assist with organizing of incoming medical records and reports.
  • Perform additional duties as assigned by a supervisor.

Qualifications

  • Completion of a practical nursing program is required.
  • High School diploma or GED is required.
  • LPN licensed in Missouri required.
  • Successful completion of the accredited Community Health Worker program is mandatory.
