Program Manager - ECM

Responsible for developing, implementing, monitoring and evaluating the Enhanced Care Management (ECM) program. Provide organizational support, program assessment, compliance and performance improvement processes consistent with the mission and goals of the organization.

• Provide direct supervision of project staff including personnel administration, work assignments, recruitment and hiring, reviews, disciplinary action and training activities.
• Prepare training curriculum and provide on-going training for project staff, clinic staff and program participants as required
• In coordination with Clinic public relations staff, develop press releases and promotional material as needed
• Participate in contract review, negotiations and amendments
• Serve as a liaison and assist with collaborative support activities with community agencies, local government representatives, neighborhood residents and other Vista Community Clinic project staff
• Responsible for monitoring quality measures associated with ECM, developing strategies for improvements within the program and associated care teams
  
  
  

Minimum

• Bachelor’s degree in public health or related field
• Minimum three years' related experience
• Valid California driver’s license, current vehicle insurance and reliable transportation
  
  
  

Preferred

• Master’s Degree in public health or related field
• Bilingual English/Spanish
  
  
  

Required Skills/Knowledge/Abilities

• Knowledge of available local community resources and government, community health centers, public assistance programs and state/federal regulations
• Ability to work flexible hours, including evenings and weekends as required
• Proficient in MS Office programs, spreadsheet, desktop publishing programs and business e-mail communication systems
  
  
  

Salary Range

• $70,000 - $80,000 DOE
