GENERAL COMPETENCIES

- ESSENTIAL JOB FUNCTIONS
- Referral Management:
- Coordinate and manage patient referrals from initial contact through to the appointment with the specialist.
- Ensure all necessary documentation and patient information is accurately collected and submitted.
- Track the progress of referrals and follow up as needed to ensure timely completion.
- Serve as the primary point of contact for patients, healthcare providers, and specialists regarding referral status.
- Communicate effectively with patients to explain the referral process and provide updates.
- Liaise with healthcare providers to clarify referral details and obtain additional information if required.
- Maintain and update referral records in the electronic health records (EHR) system.
- Generate reports on referral trends, volumes, and outcomes for management review.
- Ensure confidentiality and security of patient information in compliance with HIPAA regulations.
- Identify opportunities for improving the referral process and implement solutions to enhance efficiency and patient experience.
- Develop and update referral policies and procedures as needed.
- Provide training and support to staff on referral processes and best practices.
- Supervise and mentor a team of referral coordinators.
- Assign tasks and monitor team performance to ensure high standards of service are maintained.
- Conduct regular team meetings to discuss progress, challenges, and areas for improvement.
- Excellent organizational and multitasking abilities.
- Strong communication and interpersonal skills.
- Proficiency in using electronic health records (EHR) systems and other relevant software.
- Knowledge of HIPAA regulations and patient confidentiality standards.
- Ability to work independently and as part of a team.
