Job Overview

City Net is seeking a dedicated and compassionate Case Manager I (CM I) to deliver essential case management and supportive services to individuals and families facing street-level homelessness. In collaboration with the Riverside Police Department and Code Enforcement, our case managers engage in street outreach efforts aimed at connecting unsheltered community members to sustainable housing solutions. By meeting clients in their current environment, CMs help them set and achieve personal goals, utilizing problem-solving abilities, empathy, and de-escalation techniques to foster a safe and secure atmosphere. Our housing-first and harm-reduction approaches guide our work, ensuring clients are swiftly linked to necessary support services, including healthcare and counseling.

Key Responsibilities

• Conducting outreach to connect clients experiencing homelessness with housing resources.
• Supporting clients in the development and execution of long-term housing strategies.
• Collaborating with law enforcement, nonprofits, and various service providers to ensure comprehensive care coordination.
• Maintaining accurate and timely records of client interactions and data entry.
• Engaging clients in diverse community settings, including informal encampments, shelters, and public spaces.
• Upholding a professional presence while delivering services in challenging outdoor environments.

Qualifications

• Prior experience in homelessness services or related fields is advantageous.
• Bachelor's degree or equivalent in Social Work, Human Services, Sociology, Psychology, or a relevant discipline preferred.
• Must possess a valid CA Driver’s License and be at least 25 years of age for vehicle operation.
