We are seeking a dedicated and compassionate Home Care Nurse to become an integral part of our healthcare team. This role involves providing comprehensive nursing care in patients’ homes, ensuring adherence to their personalized care plans and addressing any concerns raised by family members and caregivers.
Key Responsibilities
- Conducting patient visits to monitor health conditions and implement care plans as directed by physicians.
- Administering prescribed medications and performing essential medical tests, including blood pressure and glucose assessments.
- Providing wound care, personal hygiene assistance, and addressing any physical or emotional needs of the patients.
- Educating family members and caregivers on effective aftercare practices and responding to their inquiries.
- Collaborating with healthcare professionals to establish improved dietary and care plans for patients.
- Compiling and maintaining accurate reports regarding patient recovery for physician review.
- Staying updated on advancements in healthcare practices and participating in relevant workshops or training.
- Exceptional observational and analytical problem-solving abilities.
- Proficiency in both following detailed instructions and exercising independent judgment.
- Strong written communication skills for effective report writing.
- Active listening capabilities coupled with a high degree of empathy.
- Physical stamina and emotional resilience to manage diverse patient needs.
- A valid driver’s license and reliable transportation may be necessary.
- A Bachelor's degree in Nursing or a related field is required.
- Relevant nursing license and certification must be obtained.
- Completion of an apprenticeship or equivalent experience in a similar capacity is preferred.
