Job Summary:

The City of Greeley is seeking a dedicated Homeless Solutions Outreach leader, who is passionate about Homeless Solutions and Community Outreach Resources.

The Outreach division helps people transition out of homelessness and poverty by providing a continuum of housing enriched supportive services. They value an individualized approach, and tailor their services/resources to meet each program participant’s unique needs.

The leader in this role will be responsible for coordinating and overseeing daily activities of Outreach Case Managers and providing oversite of the utilization of outreach services and resources within the program. They will be responsible for ensuring best practices, such as Housing First, Harm Reduction, and Trauma Informed Care etc., are utilized and incorporated at all levels of the team’s functioning. Primary duties include to engage homeless via street outreach activities, coordinate referrals and program-linkage to additional programs serving homeless, maintain thorough and accurate records in both written and electronic form, and collaborate with other homeless outreach services within the community. The position must be able to provide guidance, direction, and clinical support services, while acting as a leader and proactive team member, providing exceptional outreach services.

Finally, this position ensures that all housing programs are operating in compliance with current federal, state, and local housing and funding policies. While ensuring all staff complete their work in a consistent manner in accordance with the City’s Mission Statement and Philosophy of Service.

Experience, Knowledge, Skills:

Minimum Requirements

  • Bachelor’s degree in support services or related field of study.
  • Five (5) years mental/behavioral health direct care experience.
  • Two (2) years of supervisory experience.
  • Two (2) years of experience in managing Housing First Intensive Case management.
  • Two (2) years' experience working with and servicing people with very complex needs, including but not limited to chronic homelessness, serve trauma, substance use disorders, mental illness.

OR

  • Any combination of related education, experience, certifications, and licenses that will result in a candidate successfully performing the essential functions of the job.

Preferred

  • Bilingual in Spanish and English.
  • Master’s degree in support services or related field of study.
  • Six (6)+ months of experience in Oracle HCM.
  • Six (6)+ months of experience working within a municipality.
  • Three (3) years’ experience in managing Housing First Intensive Case management.
  • Three (3) years’ experience working with and servicing people with very complex needs, including but not limited to chronic homelessness, serve trauma, substance use disorders, mental illness.
  • One (1) year of Housing First Services experience.

Essential Functions:

  • Responsible for ensuring best practices, such as Housing First, Harm Reduction, and Trauma Informed Care etc., are utilized and incorporates at all levels of the team’s functioning.
  • Engages homeless via street outreach activities, coordinates referrals and program-linkage to additional programs serving homeless, maintains thorough and accurate records in both written and electronic form.
  • Collaborates with other homeless outreach services within the community.
  • Provides guidance, direction, and clinical support services, while acts as a leader and proactive team member.
  • Uses “best practice” methods to engage homeless persons to encourage them to accept housing placement.
  • Assists clients with immediate needs including food, water, and emergency shelter.
  • Conducts triage to determine and assist with immediate client needs including food, water, and emergency shelter.
  • Provides outreach services focused on the goal of moving homeless persons into permanent and thereby, ending their homelessness.
  • Represents the City at homeless services meetings systems meeting, as requested, cultivating, and maintaining beneficial partnerships with all providers.
  • Establishes, enhances, and implements housing intake and assistance policies and procedures across Greeley and ensures that housing programs comply with all applicable federal, state, and local regulations.
  • Serves as the point person for the local Housing Authority, Colorado State Department of Local Affairs, and Division of Housing (DOH).
  • Monitors the outcomes for all grants to ensure that the City is meeting outcomes outlined in contracts.
  • Works in collaboration with other City departments on the day-to-day monitoring of all housing program budgets.
  • Develops/maintains training for all team members.
  • Provides quality assurance and program evaluation, ensuring that programs provided are effective, efficient, and in keeping with the City vision and culture.
  • Develops and integrates new housing programs while meeting organizational and grant requirements.


Supervisory Responsibilities
