Description

Overview: Community Action Marin (CAM) is seeking an experienced and visionary Director of Health and Enhanced Care to lead strong and coordinated health support services across the agency and launch CAM’s Enhanced Care Management effort. This agency leader will ensure effective care coordination to qualifying clients of all ages and provide delivery of high-quality health services across the agency’s Children and Family Services sites, coordinating an interdisciplinary team of staff, consultants, parents, and community partners.

This position will play a transformational role in revolutionizing CAM’s care management services to ensure the highest level of support and assistance to community members and achieve impact at individual/family, agency, and community levels.

Responsibilities

Care Coordination Design, Planning and Implementation

Oversee the design, planning and successful implementation of all of CAM’s care coordination efforts

  • Develop and execute a comprehensive strategy for planning and launching CalAIM Enhanced Care Management across the agency, leading all aspects of the project and overseeing the execution of key deliverables to ensure successful implementation with key populations in community.
  • Ensure successful coordination of health services for children and families served through the agency’s early childhood education teams and sites, leading work in partnership with an interdisciplinary team of staff, consultants, parents, and community representatives
  • Supervise Health Managers to ensure dental, vision, and hearing screenings are held across all early childhood care sites and Family Child Care Homes, meeting all Head Start Performance Standards and maintaining rigorous monitoring, tracking, and compliance.

Partnership and Collaboration

Drive both internal and external collaboration efforts in service to high-quality and impactful care coordination.

  • Participate in the annual community needs assessment and update the Health Services area of the Integrated Work Plan with the Parent Leadership Council, staff, parents and appropriate community members.
  • Convene an interdisciplinary team of staff, department directors, executive leadership and consultants focused on care initiatives from intake to outcomes in order to build shared understanding of compliance requirements and set a visionary path forward to arrive at a best-in-class agency system that delivers results in community.
  • Build and lead the Health Advisory Committee (HAC) meeting and exceeding Head Start Performance Standards, convening partner organizations, health experts, community representatives, and parents. The HAC under the Senior Director’s leadership will drive the development of formal care coordination partnerships, data sharing agreements, informal collaboration, and advocacy opportunities to benefit Marin residents.

Quality Assurance and Compliance

Develop protocols for monitoring and evaluating the quality and effectiveness of all care coordination services.

  • Working with Learning and Evaluation, drive the collection and analysis of data and utilize data driven insights to implement continuous improvement efforts and ensure the highest level of service delivery
  • Partners with Operations, Finance, and contracted partners to ensure compliance with all relevant regulations, policies, and procedures governing services, financial management, and data sharing
  • Stay informed of changes in regulations and best practices through active participation in regional and state committees and meetings

Team Leadership and Development

Provide strong leadership and guidance to a multidisciplinary team of care, fostering a culture of collaboration, innovation, and excellence.

  • Serve as a resource to staff regarding health issues and needs, including oral health, and physical health; work with the mental health and nutrition staff and consultant resources to ensure a site-specific delivery that is uniform and targeted to meet family needs.
  • In partnership with Learning and Development, develop and implement training programs to equip staff with the knowledge and skills necessary to effectively carry out their roles, comply with state and federal requirements and drive the new enhanced care coordination framework.
  • Supervise Enhanced Care Managers working across CAM with assigned caseload and supporting efforts to measurably improve health outcomes of clients.
  • Attend and actively engage in required meetings, including CFS Directors, Leadership Team, People Managers, and Programs Team meetings.

Qualifications

Qualifications:

  • Bachelor's degree in healthcare administration, public health, social work, or a related field; Master's degree preferred.
  • Minimum of 5 years of experience in healthcare management, with a focus on care management or care coordination.
  • In-depth understanding of the CalAIM Enhanced Care Management process and related initiatives.
  • Proven track record of successful project management, including planning, execution, and evaluation.
  • Strong leadership and team-building skills, with the ability to inspire and motivate staff towards achieving common goals.
  • Excellent communication and interpersonal skills, with the ability to effectively engage with stakeholders at all levels.
  • Experience with data analysis and performance measurement, with a focus on using data to drive decision-making and quality improvement efforts.
  • Knowledge of relevant regulations, policies, and procedures governing care management services.
  • Commitment to equity, diversity, and inclusion, with the ability to work effectively with individuals from diverse backgrounds and communities.
