Your Role on the Team

Position Summary

The individual in this position works under the direction of the RN and/or Social Worker to complete referrals for post acute services for patients and assist with tasks that do not require a clinical license or degree. The individual’s responsibilities include the following activities:

Required Qualifications

• Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
• Must demonstrate customer service skills appropriate to the job
• Excellent written and verbal communication skills in English
• Ability to maintain a work pace appropriate to the workload
• Ability to establish and maintain effective working relationships across the Health System
• High School diploma or equivalent required. Bachelor’s degree preferred. Paramedic, EMT or Nursing Assistant license preferred. Acute hospital experience preferred. Skills required include excellent organizational skills, excellent verbal and written communication skills, demonstrated problem solving skills, and computer literacy.
