About The Role
The Outreach Specialist is owner/driver responsible for successful enrollment of prospective participants into the PACE program, by developing and building relationships and generating quality participant referrals that lead to enrollment. The Outreach Specialist is a member of the Marketing, Outreach, and Enrollment (MOE) team and provides back-up and support to Outreach and Enrollment functions as assigned. Collaborates effectively with colleagues and stakeholders to promote the Seen Health values, team culture and mission.
Responsibilities
Education/Training/Certifications:
Required that Team Members must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact. Additional screening may be required in order to protect participants and the community from communicable disease outbreaks (e.g., COVID-19).
Regulatory Requirements
Must not be on any exclusion or debarment from participation in Federal Health Care Programs at any time and must remain in good standing with government regulators such as the OIG, (Office of the Inspector General), SAM, (System for Award Management), CMS (Centers for Medicare and Medicaid Services), etc.

About The Role

Responsibilities
- Coordinate and execute outreach, PR and community engagement activities to ensure consistent achievement of monthly referral, home visit and enrollment goals.
- Identify opportunities to collaborate with community organizations and execute against those opportunities in a thoughtful manner that reflects our mission and value proposition.
- Establish and maintain best in class relationships with community leaders and partners including physicians, hospital discharge planners, post-acute facilities and other potential referral sources in the service area.
- Develop and implement annual marketing plan to achieve monthly enrollment goals with MOE Director.
- Provide education to referral source in the community regarding Seen Health services.
- Demonstrate in-depth knowledge of all relevant components of MOE Playbook.
- Comply with all rules pertaining to allowable and prohibited marketing practices.
- Identify multiple potential opportunities for process improvement.
- Must be willing and have the ability to work a varied schedule that may include evening nights, weekends and overtime.
- Complete all required documentation and generate reports in a timely and accurate manner.
- Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
- Follow Seen Health policies and procedures and participate in any required Quality Improvement activities, staff training and meetings.
- Communicate regularly with Supervisor and team regarding workload and priorities.
- Timely completion of all mandated trainings and education.
- Timely completion of all mandated occupational health screenings as needed.
- Exercises flexibility in performing assignments as business needs evolve.
- Other duties as assigned.
Education/Training/Certifications:
- Bachelor’s Degree Marketing or Health Care Administration preferred.
- DHCS Training and Exam as a marketing representative within 30 days of hire.
- Valid State Driver’s License with a clean DMV record.
- Minimum of two years’ experience in a successful sales and marketing role with at least one year in healthcare or senior care setting. Within service area preferred.
- Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.
- Patience, with a “can do” attitude.
- Consistent attention to detail on daily deliverables and small to medium sized projects.
- Familiarity with local community agencies and resources preferred.
- Excellent organizational and communication skills.
- Ability to work independently with minimal supervision.
- Demonstrated ability to prioritize in a fast-paced environment.
- Experience and competency working with people from diverse backgrounds and cultures.

- Regular travel to different settings in the San Gabriel Valley community.
- In center at Seen Health in Alhambra, CA.
