Exciting News: We're Expanding into new territories and hiring for a Chaplain to serve across Winnebago and Outagamie counties. The Chaplain provides spiritual support or arranges support to meet the spiritual needs of patients and families in a manner consistent with their beliefs and desires and in accordance with the patient’s plan of care.

Schedule

Part-time 27 hours per week, 3 days per week at 9 hours per day. The days can be flexible you would need to work every other week on Thursday to attend IDT meetings.

Additionally: 1 weekend every other month. Weekend hours are; Friday's from 5-7pm, Saturday's 9am-5pm and Sunday 10-4pm. On-call rotation & 1 holiday per year working 9a-5p.

Essential Job Duties

  • Provide direct spiritual counsel to patients/families as requested:
  • Completes initial spiritual assessment
  • Contacts the patient’s clergy as requested by patients and family
  • Establishes visit plan and spiritual plan of care
  • Provides prayer and sacraments to patients/families as requested
  • Coordinates care with patient’s clergy when requested
  • Is alert to the theological/ethical issues that may be of concern to patients/families and team members
  • Assists in planning funeral services, prior to death, as requested by the patient’s family
  • Serves as liaison with the local religious community:
  • Provides support and counsel to clergy working with patients/families admitted to Unity's programs
  • Provides an awareness of hospice and palliative services and death education to the religious community
  • Maintains contact with the continuing education resources for clergy and religious caregivers in the area
  • Attends IDT meetings:
  • Participates in the development of the initial plan of care, and provides input into the review and revision
  • Serves as a resource to other patient-care staff on patients'/families spiritual belief systems
  • Collaborates with the bereavement team in the planning and offering of memorial services and programs
  • Participates in process improvement projects to enhance the spiritual care services provided to patients and families.
  • Works with the volunteer services department to place spiritual care volunteers with appropriate patients and families

Requirements

Qualifications

  • Bachelor's degree from an accredited school of theology, religious studies or related studies and/or certification in hospital or pastoral ministry required. Master's degree in theology, divinity or pastoral studies preferred
  • Board Certification by the NACC (National Association of Catholic Chaplains or APC (Association of Professional Chaplains) or working toward certification
  • Two units of Clinical Pastoral Education preferred
  • Experience in hospice and palliative care and an understanding of hospice and palliative philosophy is preferred
  • Valid Wisconsin Driver’s License, reliable transportation, and proof of current automobile insurance coverage
  • Internet access to accommodate charting and laptop synchronizing capabilities
