Key Responsibilities

• Collaborate with therapy and facility teams to deliver exceptional care.
• Educate families and caregivers on best practices and therapy procedures.
• Participate in in-service training and facility education sessions.
• Attend Care Plan Meetings to contribute to resident care strategies.
• Maintain accurate daily documentation of therapy interventions.
• Work closely with the supervising Physical Therapist to ensure aligned care plans.
